Insurance & Payment
Tallmadge Family Eye Care does not want cost to be a factor when it comes to your eye health. We accept most major insurance plans, as well as flexible spending accounts and health savings accounts. We also accept Care Credit, personal checks, money orders, cash, and most major credit cards.
About Your Insurance
There are two types of health insurance that will help pay for your eye care services and optical products. Major medical health insurance helps cover the cost of services related to eye injuries and diseases while vision insurance provides coverage for routine eye exams and prescription eyewear, such as frames, lenses, and contact lenses. We accept both Medical and Vision insurance plans.

We understand the financial concerns that often come when visiting the eye doctor which is why we thoroughly review all the insurance benefits available to you to help you get the most out of your benefits and minimize your out-of-pocket expenses. If you have both types of insurance plans it may even be necessary for us to bill some services to one plan and some services to another. We will follow a procedure called coordination of benefits to do this properly and to lower patient responsibility. If some fees are not paid by your insurance, we will bill you for them, such as deductibles, co-pays, or non-covered services as allowed by the insurance contract.

Please provide your insurance cards to one of our staff members so we can make a copy to have on your file. It is important we have your most up-to-date medical insurance or Medicare card and vision insurance on file for all billings to your insurance to avoid any potential processing or payment issues.

If you have questions regarding your visit, copays, or any fees, contact our office and one of our staff members will work with you to help you understand your insurance benefits and all other options available to you. We believe that lack of insurance should not keep you from quality health care. We offer discounted eye exams, glasses, and contact lens fittings for patients without insurance.
