Patient Portal

Our online patient portal allows you to access copies of your glasses or contact lens prescriptions, and send secure comments, questions, or concerns to your provider or our staff

In order to access our secure online portal, you must register for an account with us. Please contact us to request your access to these secure resources. We will email you the username for your account, as well as a link to access the portal. Before logging in, you will need to reset the password to create the password of your choice.

How do I sign up?

Please contact our office or ask us for a portal account at your next appointment. We will create a username for your and send you an email with your username and link to the portal. Before logging in, you will need to reset your password to create the password of your choice. We cannot create a password for you. 

I am having trouble setting up my portal account. What should I do?

If you are having trouble setting up your portal account, please first check your spam folders to make sure your username and/or password emails did not accidently go to your spam. If you are still having trouble or unable to find the emails in your inbox or spam folders, please contact us for assistance.

How do I change my password?

To change your password, please login to your portal account. On the side menu, select "Account". From there you will have the option to change your password.

Is there a fee to use the patient portal?

No. The patient portal is a completely free way to access secure resources regarding your health. 

I forgot my username. What should I do?

On the login screen, choose "Forgot Username". Enter your email address and your username will be emailed to you. 

I forgot my password. What should I do?

On the login screen, choose "Reset Password". Enter your username and a password reset link will be sent to the email address we have on file. If you have trouble resetting your password, please contact us for assistance.

I have too many failed login attempts. My account has been disabled. What should I do?

Please contact our office to reset your account.
