At Tallmadge Family Eye Care, we want everyone to feel comfortable and cared for during their visit. We offer a friendly environment that caters to all walks of life. We are passionate about healthcare and want you to see your best.
The first step towards keeping your eyes healthy is to schedule an appointment. Please contact our office by phone, stop in, or click here to schedule an appointment online.
Did you know?
Your eye exam is an important part of your overall health and typically includes: an eye health check, a refractive exam, dilation, and a prescription for glasses or contact lenses.
What to Bring
Please come to your appointment with:

Your current glasses and prescription sunglasses.

Your ID, health insurance cards and vision insurance card (if you have one).

A list of current medications, including vitamins and supplements, and any over the counter medications.
If you wear contact lenses, please bring:

Any contact lens blister packs or boxes, along with your current glasses.

If you are a new patient, please bring a copy of your current contact lens prescription, if you have it. A refitting may be necessary if we cannot verify your brand.

Wear your contacts to your appointment. If your contact lenses are uncomfortable for any reason, remove them and wear your glasses instead.
About your Insurance
There are two types of health insurance that will help pay for your eye care services and optical products. We accept both Medical and Vision insurance plans. If you have both types of insurance plans it may be necessary for us to bill some services to one plan and some services to another. We will follow a procedure called coordination of benefits to do this properly and to minimize your out-of-pocket expenses. If some fees are not paid by your insurance, we will bill you for them, such as deductibles, co-pays, or non-covered services as allowed by the insurance contract. Please provide your insurance cards to one of our staff members so me can make a copy to have on your file. We need to have your medical insurance or Medicare card on file for future billings to your insurance.
Previous medical records are generally not needed. However, if you have been treated for an ocular medical issue or are seeking ongoing treatment, please bring your previous doctor's contact information. We may request your previous medical records for reference.
